By completing this form, your child's name and submitted photo will be included in the Survivor Families Recognition Moment during Aaliyah Day 2026 on Thursday, July 2, 2026, at Sawmill Creek Park in Glen Burnie, Maryland. The recognition ceremony is scheduled to take place at approximately 7:00 PM.
